All students starting or transferring into 6th grade are required by law to have an additional dose of Diphtheria, Tetanus and Pertussis (DTP) vaccine and, after your child's 11th birthday, one dose of the Meningococcal (meningitis) vaccine.
If your child is starting or transferring into the 6th grade, but will not be 11 years old until after the start of the school year, this requirement will be waived until your child turns 11 years old. You will need to have these immunization requirements met immediately after your child's 11th birthday for them to be allowed to remain in school.
I will need a doctor's certificate documenting the dates of both immunizations. All students must be in compliance with the New Jersey State immunization regulations. These vaccines must be obtained before the start of school in September.
